The forms listed below are needed to participate in all Maple Athletic/Activity Programs.

Updated Physical Forms
For student-athletes entering 6th Grade, the “Certificate of Child Health Exam” Form required for all incoming students in 6th Grade can serve as the athletic physical. Make sure that your physician checks the interscholastic sports participation box. For students entering 7th and 8th Grade, the athletic physical form is the "IESA Pre-Participation Examination” Form. In any case, the physical must be less than a year old.

If these forms are not submitted a student-athlete is not allowed to practice or participate with their team(s) in any games, meets, events, etc.
